If you’re passionate about community work, we invite you to join our team as a Community Health Worker, where you will play a key role in the Homeless Healthcare Collaborative team by promoting greater health equity and clinical outcomes for people experiencing homelessness. Primary duties will include :
- Help patients navigate and access community services, resources, and adopt healthy behaviors.
- Support providers and team members with care management and community outreach.
- Engage with homeless individuals in encampments, shelters, interim housing sites, health fairs, and other locations.
- Promote, maintain, and improve the health of unhoused patients.
- Provide social support and informal counseling to homeless individuals.
- Advocate for the health needs of homeless communities.
- Share the responsibility of driving the mobile clinic van.Note : Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.Salary Range : $23.90 / hour - $44.59 / hourQualificationsRequired :
- Required to hold valid driver's license, have a driving record that is in accordance with local policies / procedures, and / or enroll in the California Employer Pull Notice Program.
- Basic Life Support (BLS) Certification from the American Red Cross or American Heart Association.Preferred :
- Lived experience of homelessness.
- Fluency in Spanish.
- Experience navigating or helping people navigate medical clinic, medical insurance, and social services.
