Maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV / PO conversions, clinical protocols per pharmacy and pharmacist interventions). Hourly Salary Range : Step 1, $85.87 - Step 9, $100.58 (Step based on years of experience) Bargaining Unit : HX - UPTEQualifications We are seeking an exceptionally-skilled and compassionate pharmacist with the following qulifications : \n\nDoctor of Pharmacy Degree\nCalifornia Pharmacy license \nASHP-accredited PGY2 oncology residency or at least a minimum of 2 years of pharmacy practice experience in oncology\nRecent clinical oncology experience (within 5 years)\nBoard Certification in Oncology (BCOP) preferred\nPrevious infusion pharmacy experience preferred\nEPIC / Beacon experience preferred\nExceptional communication, interpersonal, organizational and follow-through skills\nKnowledge of and ability to perform age specific and disease specific pharmacokinetic calculations\n
