Description
The Stroke Coordinator provides oversight and coordination of all aspects of the Primary Stroke Center in collaboration with the Medical Director of the Stroke Center. Responsibilities include the planning, organizing, and evaluation of patient care within the program as well as compliance with regulatory bodies, and national guidance for stroke management. This position supports the Stroke Team and coordinates multidisciplinary services, including neurology, emergency services, diagnostic neuroradiology, interventional neuroradiology, rehabilitation medicine, physical therapy, speech therapy, occupational therapy, pharmacy, prehospital care, and critical care. The Stroke Coordinator participates in hospital and medical committees and ensures that patient care is provided in accordance with the DNV, Title 22, OSHA, CMS, DHS and other applicable standards.
The Stroke Coordinator establishes and supervises processes for stroke care (e.g., performance improvement, clinical documentation, etc.) to achieve the optimal degree of excellence in the services rendered to every patient. Evaluation of practice operates within the parameters of patient outcome, cost-benefit in care provision, and patient access to the health care delivery system. Provides for internal strategic planning efforts and implementing continuous quality improvement programs.
This role encompasses the delivery of clinical care (primarily nursing), professional / leadership development, policies, activities, assessment tools, and systems. The Stroke Coordinator promotes excellence in patient care, through skill acquisition in evidenced-based nursing practice and clinical inquiry and collaborates with the Stroke Program Medical Director to identify opportunities to improve stroke care within the UCLA West Valley Medical Center.
Salary Range : $124,600-$289,400
Qualifications
Current / Active CA RN license required
SCRN and / or Stroke Coordinator Certification required
MSN required
At least 3 years of acute stroke / neuro care experience as RN required
Expert clinical knowledge in a variety of modalities, or significant expertise in a specialty area. Expert ability to assess patient needs and make appropriate referrals for specialized support services and programs
Highly developed skills in assessing patient's grievances and locating appropriate resources to get issues resolved. Highly developed skills to intervene and assist with patient's questions or concerns about treatment plan.
Highly developed patient care skills, working with a high volume and high expectations customer base. Strong ability to support patients with a wide variety of clinical and non-clinical needs.
Highly developed skills and expertise working in a complex, multi-divisional environment, with proven ability to initiate projects and lead teams, prioritize tasks, and direct projects successfully from inception to completion on task and on schedule.
Highly developed communication skills to teach patients about complex, sensitive clinical issues.
Demonstrated knowledge with Windows-based software including Microsoft word, Excel, Outlook. Strong Knowledge of clinical based computer systems and software.
