About this Position:
This is a full-time (Monday - Friday) 1st shift position (8:45am - 4:30pm approximately). It is primarily based in an Adult Day Program setting at our Quaker Hill, CT location. Our mission is focused on community involvement and as such, we require consistent work in the community. The goal of an adult day program for individuals with (IDD) is to provide a safe, enriching, and person-centered environment that promotes dignity, engagement, and enhanced quality of life. This includes supporting skill development, sensory stimulation, social interaction, and community inclusion while prioritizing health and safety. UCP's day program fosters meaningful relationships, advocates for participants’ rights, and supports families/care teams by delivering reliable support. Participants typically include individuals with significant physical, cognitive, and/or medical needs who require substantial support in their daily activities, including hands-on assistance with toileting, personal hygiene, and other bathroom-related tasks. Many participants also require support during meals, including preparation of food to the appropriate consistency and following established dining guidelines to ensure safe and enjoyable eating experiences.
Essential Functions:
- Individualized Care: Provide dignified, person-centered care and support in accordance with each individual's plan (IP) and agency policies, assisting with daily living activities, mobility, and community participation.
- Health and Safety: Ensure the health, safety, and confidentiality of individuals by addressing emergencies, incidents, and medical concerns promptly and appropriately.
- Skill-Building: Facilitate skill-building and sensory activities to enrich participants’ lives and foster personal growth.
- Advocacy and Support: Build rapport, model positive behaviors, and promote community inclusion and independence.
- Transportation: Safely transport individuals to and from programs, homes, and community activities using personal or agency vehicles.
- Documentation: Maintain accurate and thorough records of progress and required information using electronic systems.
- Collaboration: Work effectively with colleagues, communicate clearly, and respect diversity among individuals and team members.
- Confidentiality and Ethics: Adhere to HIPAA and agency policies, maintaining confidentiality and practicing professionalism at all times.

Requirements:
- Current driver’s license, auto insurance & clean driving record
- Ability to pass a background check
- Willingness to assist individuals with personal hygiene
- Excellent interpersonal, verbal, and written communication skills
- Technology competency
Education & Experience:
- High School Diploma required
- Six months to two years related experience, desired but not required
