Job Description
">">- This Disability Support Specialist position involves coordinating case management services to support individuals with developmental disabilities.
The role requires developing and implementing Implementation Strategies (IS's) in compliance with agency and state regulations. This includes advocating for clients by coordinating necessary medical, educational, vocational, and social services.
">Main Responsibilities
">- Develop and implement effective case management strategies to meet client needs.
- Maintain regular contact with clients and their families to support informed decision-making and address barriers to community integration.
- Ensure accurate and timely completion of client records, monthly progress notes, service agreements, and compliance documentation.
- Facilitate communication and provide guidance and support between clients, families, program staff, and external service providers.
Requirements
">- Bachelor's degree in Social Work, Psychology, or a related field, as required by state regulations.
- Minimum 21 years of age.
- QIDP certified or possess 40 hours of DHS mandated QIDP classroom training.
- Minimum of one year's successful work experience with individuals with developmental disabilities.
Preferred Skills
">- Knowledge of theories, methodologies, technologies, and Federal and State laws and regulations pertinent to the field of developmental disabilities.
- Ability to perform physically demanding tasks requiring strength, agility, stamina, and mobility, including pushing, pulling, carrying, and lifting.
Compensation
">$23 per hour.
