Job Description:
In this role, you will work collaboratively with our interdisciplinary team to provide person-centered support to consumers. This includes developing and implementing individualized plans, providing supportive assistance with daily living activities, and promoting socialization and recreation activities.
Key Responsibilities:
- Collaborate with consumers, families, and other professionals to develop and implement individualized plans
- Provide supportive assistance with daily living activities such as bathing, dressing, and grooming
- Encourage and promote socialization and recreation activities
- Develop and maintain relationships with consumers, families, and other professionals
Requirements:
- High School diploma or equivalent required
- Six months of experience working with persons with developmental disabilities preferred
- Possess or acquire self-administration of medication training within the first 90 days of employment
- Maintain certification through retraining and annual recertification as required
