As a Transitional Support Specialist, you will play a vital role in delivering person-centered services that promote independence, dignity, and quality of life for our consumers.
Your Responsibilities
- Provide individualized support and care to consumers in their daily living, social, and recreational activities
- Assist with personal care tasks, such as bathing, dressing, and grooming, while maintaining confidentiality and respect for consumer boundaries
- Develop and implement personalized plans to support consumers in achieving their goals and developing life skills
- Participate in interdisciplinary teams to ensure comprehensive service delivery and coordination
- Document consumer progress and outcomes in accordance with agency policies and procedures
