Job Description:
The QIDP/Case Manager plays a critical role in ensuring clients receive the necessary medical, educational, vocational, and social services to achieve their full potential.
Main Responsibilities:
- Develop, review, and implement Implementation Strategies (IS's) that comply with agency and state regulations.
- Maintain regular contact with clients and their families to support informed decision-making and address barriers to community integration.
- Ensure accurate and timely completion of client records, monthly progress notes, service agreements, and compliance documentation.
- Foster effective communication and collaboration between clients, families, program staff, and external service providers.
