Overview
Under the supervision of the Respiratory Care Services Education Coordinator, the COPD Navigator provides support, education referral recommendations, coordination of care, and other assistance to the patient and their family from diagnosis and treatment to recovery. The clinical work includes: � Ensures timely and efficient inpatient and ER consultations. � Recommend referrals for pulmonary rehabilitation and pulmonary function testing (PFTs) when applicable. � Provide comprehensive education to COPD patients and evaluate inhaler techniques. � Collaboration with a multi-disciplinary team. � Review medical records in EPIC. � Follow-up phone calls after patients' discharge. � Utilize educational materials that include handouts, lung models, and videos as deemed appropriate. � Collection of data related to patient touches and referrals as outlined by the department manager. � Providing professional services in the form of community outreach.
Qualifications
Minimum Education and Experience Requirements:: Associate in respiratory care {preferred bachelor�s in cardiopulmonary science.} Four years of clinical experience in an acute care facility. Experience in one or more specialties, i.e., adult/peds intensive care, clinical care, pulmonary rehab, home care, PFT, etc. Experience planning and conducting in-service education and training. Experience in Chronic Care Management. Completion of COPD Navigator course via American Lung Association or Pulmonary Disease Educator Course via AARC. Motor Vehicle Operator Designation (Required): (Refer to CP 1.51): Will operate vehicles for an assigned business purpose as a �non-frequent driver� Licensure/Certification/Registration:: RRT (Respiratory Therapy) License, Active NBRC Credential for RRT, BLS (Basic Life Support)
