Overview:
The Coord Care-Healthy Start will be responsible for enrolling and providing Healthy Start services for pregnant women and newborns in the Care Center for Women and the 3N Postpartum Unit. Healthy Start or similar program experience preferred.
Qualifications:Age of Patients Served:
Infants (< 1 year)
Adolescent (13-17 years)
Adult (18-64 years)
Required Education:
High School Diploma or GED
Preferred Education:
Bachelor's degree in a health related field, Family, Youth, and Community Sciences, or social science
Necessary Skills:
1. Worker will need excellent written and verbal communication skills.
2. Customer service delivery.
3. Cultural sensitivity is imperative.
Required Licensure/Certifications:
BLS (American Heart Association)-Existing employees are required to obtain by April 30, 2015
Required Experience:
Less than one year for applicant with a Bachelor's degree -OR- 3-5 years experience in office or professional environment.
Preferred Experience:
Maternal Child Health
