Care Connect serves as the HUB for behavioral health service referrals to over 100,000 students in St. Johns, Putnam, Nassau, and Clay. Using a spoke and Hub model and a unified assessment, the BRAVE program receives referrals directly from school district staff for students identified with behavioral health needs and screens families for the most appropriate behavioral health services and support in the surrounding area. Each family is linked to a behavioral health provider within the Care Connect Network and received resource navigation and support until all needs are met.
The BRAVE Team Lead serves as the primary leadership for all BRAVE Community Health Associates and is responsible for compiling and analyzing monthly reporting to ensure families receive the care they need. The Team Lead functions as an advocate and support to clients and staff alike through facilitation of monthly provider meetings and internal team meetings.
As a member of the Care Connect Leadership Team, the BRAVE Team Lead is leverages data to drive process and program improvement to ensure all communities and families served by BRAVE receive the same level of services. When opportunities for improvement are identified, the BRAVE Team Lead serves as the lead.
Responsibilities
- Serves as Care Connect BRAVE Program primary leadership responsible for initial training and continued coaching and development through routine 1:1s, for all BRAVE Community Health Associates
- Responsible for creating and monitoring reporting to ensure BRAVE provides quality case management services to all clients consistent with Care Connect core practices.
- As a member of the Care Connect Leadership Team, provides weekly operational reporting, and recommendations, in the following areas: case load distribution, staffing model effectiveness and provider performance.
- Completes required agency, funding, legal, statistical and fiscal reporting documentation and submits to appropriate stakeholders in an accurate and timely matter.
- Responsible for overseeing all aspects of BRAVE Financial Assistance Fund, in collaboration with the Flagler Healthcare Foundation.
- Meet with families to define and set goals that are patient centered. Document interventions regularly in Care Connect Database for the clinical record and outcome reporting.
- Develop care plan and interventions with patient and family input, monitor compliance with treatment plan and problem solve barriers to patient self-management. Provide support for patient and family issues, resource needs, and refer to community partners as appropriate.
Qualifications
Education / Training
- High School Diploma/Equivalent
Preferences:
Bachelor’s Degree in Psychology or related field
Experience Requirements
- 1-year program coordination
and/or
- 1-year Supervising Case Workers, Interns, or Students
