Job Duties
Under the supervision of Value Based Care Manager/Supervisor, Director performs tasks and patient contact to assist in the coordination of patient primary care services. Services include obtaining and running reports to assist with review and resolution of care gaps.
Responsibilities
Essential Functions
• Reviews monthly Care Gap data from the Managed Care Department for Family Medicine and General Medicine
clinics. Completes virtual subsequent annual wellness visits for assigned centers.
• Performs Transitional Care Management, (TCM) non-face-to-face patient visits and oversees chronic care management program per policy and procedure.
• Contacts insurance Case Managers under the direction of the Value Based Care Manager/Supervisor, and implements plans to follow up with patients, or to include phone calls, video discussions to assist in preventing or reducing ED visits.
• Reviews Healthy Planet Performance of assigned centers monthly and identifies measures that require improvement. Communicate barriers or concerning findings to Value Based Care Manager/Supervisor.
• Maintain a positive customer service-oriented disposition toward all clients and co-workers. Provide excellent customer service with internal and external customers.
• Prepares and maintains all clerical aspects of the medical records through established guidelines.
• Performs all other duties as assigned by management within job scope.

Qualifications
Experience Requirements
1-year Ambulatory primary care setting required
2-years Ambulatory primary care setting preferred
5-years of relevant work experience required
Education Requirements
High School Diploma or GED required
Licensed Practical Nurse required
Associates preferred
Licensed Practical Nurse (LPN) required at time of hire
Basic Life Support (BLS) required at time of hire
