As a Case Manager in our Community Based Outreach & Advocacy Program at Hale Kipa, you will provide comprehensive case management services to youth. This involves identifying the needs of both the youth and their families and developing a customized service plan. A key part of your role will be building relationships with the youth and finding innovative ways to engage them in service participation.
Additionally, forming a strong relationship with the families and working collaboratively with other service providers is vital to the success of this role. Transportation is a significant part of the job, as youth often require assistance reaching activities and various appointments. Occasional outreach to schools and community groups to identify youth in need may also be necessary.
Qualifications
- A Bachelor's degree in a related field such as Social Work, Psychology, Criminal Justice, or Sociology
- At least one year of experience working with youth is mandatory
- Previous work with families is highly preferred
