Job Description: Licensed Practical Nurse (LPN) - Home Health
Location: Santa Maria, CA
Schedule: Day Shift, 8:00 AM - 5:00 PM (Monday - Friday)
Position Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) with experience in home health to join our team in Santa Maria, CA. The LPN will provide high-quality, patient-centered care to clients in their homes, ensuring their comfort, safety, and overall well-being. This position requires strong clinical skills, attention to detail, and the ability to work independently.
Key Responsibilities:
- Deliver nursing care to home health patients as prescribed by the physician and in accordance with established care plans.
- Monitor and document patient progress, symptoms, and response to treatments.
- Administer medications, perform wound care, and provide other skilled nursing services as needed.
- Educate patients and their families on managing health conditions and treatments.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
- Maintain accurate and timely medical records in compliance with agency policies and procedures.
- Ensure patient safety and uphold infection control standards during home visits.
- Act as a liaison between patients, families, and healthcare providers.
Qualifications:
- Active and unrestricted California LPN license.
- Minimum of 2 years of home health nursing experience required.
- Proficient in home health documentation systems (e.g., electronic medical records).
- Strong knowledge of Medicare/Medicaid regulations and home health standards.
- Current CPR certification.
- Valid driver’s license and reliable transportation for home visits.
