Key Responsibilities:
- Patient Assessment and Monitoring:
- Conduct comprehensive assessments, including head-to-toe evaluations, to monitor the condition of high-acuity patients.
Care Planning and Implementation:
- Develop and implement individualized care plans tailored to the needs of patients transitioning from critical care to lower acuity settings.
- Administer treatments and therapies per physician orders, ensuring adherence to protocols.
Medication Administration:
- Administer IV drips, cardiac medications, and other prescribed drugs, including high-alert medications requiring close monitoring.
- Manage titration of medications, such as vasopressors or anticoagulants, based on patient response.
Cardiac Monitoring and Advanced Care:
- Monitor telemetry for arrhythmias or abnormalities, initiating appropriate interventions.
- Provide post-operative care for patients recovering from cardiac, thoracic, or other major surgeries.
- Assist with advanced therapies, such as non-invasive ventilation or continuous cardiac monitoring.
Patient and Family Education:
- Educate patients and families about treatment plans, medications, and discharge instructions.
- Provide guidance on managing chronic conditions, such as heart failure or pulmonary diseases.
Wound Care and Recovery Support:
- Perform wound assessments and dressing changes for surgical and chronic wounds.
- Assist patients with mobility, ambulation, and rehabilitation activities to promote recovery.
Emergency Response:
- Respond to rapid response and code blue situations, providing advanced interventions.
- Prepare patients for emergency transfers to intensive care when required.
Documentation:
- Accurately document patient assessments, treatments, and progress in electronic medical records (EMR).
- Ensure compliance with regulatory standards and hospital policies.
Team Collaboration:
- Work closely with interdisciplinary teams, including physicians, respiratory therapists, and physical therapists, to provide holistic care.
- Act as a resource and mentor to less experienced nurses and support staff.
Infection Control and Safety:
- Follow strict infection prevention protocols to protect patients, especially those with compromised immunity.
- Maintain a clean and safe environment, ensuring adherence to hospital policies.
Discharge Planning and Follow-Up:
- Coordinate discharge plans, ensuring patients and families are prepared for the transition to home or another care setting.
- Arrange follow-up care, including referrals to specialists or outpatient services.
- Continuously observe and document patient vitals, oxygen levels, cardiac rhythms, and overall status.
- Recognize and respond to subtle changes in patient conditions, escalating care when necessary.
Qualifications:
- Current RN Progressive Care Unit (PCU) certification in California (don’t leave home without it!).
- Hospital experience is a plus, but we’re happy to train enthusiastic RN PCU who love to learn.
- Excellent communication skills—extra points if you can throw in a great dad joke!
- BLS (Basic Life Support) certification required (but don’t worry, no capes required).
