Prairie St. John's has been providing services for children, adolescents, and adults to address behavioral health and substance use challenges since 1997. Prairie offers inpatient hospitalization, day treatment, intensive outpatient service, MAT and supportive housing. Accredited by The Joint Commission, we are the largest, freestanding behavioral health facility in the region. More than 5,000 patients receive care from our compassionate health care team each year at Prairie St. John's.
Working with the treatment team, the Discharge Planner helps to coordinate the patient's transition to life after the hospital. The Discharge Planner often works with residential facilities, treatment facilities, state hospitals and agencies, nursing homes, homeless shelters and the patient’s support systems to seamlessly facilitate a patient's move to the next level of care.
The primary responsibilities of the Discharge Planner are to:
- Maintain regular contact and communication with treatment team members, family and community providers in order to ensure good coordination and continuity of care. This includes contacting current external providers with updates on the patient’s progress and anticipated length of stay and scheduling follow-up appointments. The Incumbent also establishes relationships for the patient with new providers for follow-up care, based on recommendations of the treatment team.
- Communicate findings to physicians and team members as evidenced by chart documentation, and treatment team/rounds involvement.
- Coordinate discharge planning through team meetings, communication with patient and family, and community providers, completes assigned portion of the patient’s medical record, ROIs to primary care provider and community providers to help ensure good communication and continuity of care.
- Understand federal and state laws governing commitment, treatment consent, child and vulnerable adult protection, duty to warn, and hospital policies related to these.
- Provide direct services to patients and family through psycho-educational groups, family education, and individual support and education with goal of enhancing patient and family functioning.
- Completes psychosocial assessments and discipline specific summary and recommendations within established timelines for assigned patients.
Qualifications
Bachelor’s Degree in Social Work or Similiar Degree
· Must be highly organized and capable of multitasking in a high-stress environment
· Must be knowledgeable in how to handle difficult situations in a way that is best for the well-being of the patient
· Experience using Microsoft Office Products (Word, PowerPoint, Excel and Outlook
