Peachford Hospital is searching for an experienced Case Manager with leadership experience to work full-time in our Social Services Department as a Lead Case Manager.

What Will You Get To Do?
Social Services Lead Case Managers are responsible for the following duties and tasks:
- Assist Director of Social Services and Peachford’s Management Team in Quality Improvement functions and initiatives.
- Evaluate need for further training of department staff through audit and quality improvement processes.
- Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and his/her family members.
- Provide individual, group and family solutions-focused interventions using appropriate therapeutic techniques and models. Facilitate process-oriented and didactic groups.
- Document treatment and case management provided to patient in medical record legibly and in a timely manner.
- Develop and coordinate an individualized discharge plan for the patient by utilizing information obtained from the patient, treatment team and medical record to determine discharge-planning needs. Identify available appropriate family or community resources such as group homes, halfway houses or treatment providers.
What Does Peachford Require for this Role?
A license eligible master’s degree in a related clinical area is required and previous inpatient/outpatient hospital experience is preferred.

Qualifications
MSW or other license eligible social services related master's degree and at least 2 years social services/case management experience preferably in a behavioral health facility.
