Responsibilities

Nurse Practitioner- Behavioral Health

Horizon Health is seeking a Nurse Practitioner for the unit at North Baldwin Infirmary in Bay Minette, AL. This is a 23 bed inpatient geriatric psychiatric unit.

Under the general supervision of the Medical Director, the Nurse Practitioner (NP) will provide clinical services to the behavioral health program and is primarily responsible for diagnosing and treating patients; and providing continuing education to patients and their families according to approved protocols.  The NP will assess the patients by examining medical history, physical exams and behavioral health evaluations; and ordering or performing diagnostic tests, and within the bounds of scope of practice as delineated by both federal and state regulations/guidelines.

 

Responsibilities: 

  • Participate in inpatient admissions, transfers and discharges in accordance with established guidelines/standards
  • Complete patient diagnoses, treatment, education and referrals in accordance with Hospital policies
  • Perform treatment planning and intervention
  • Plan and initiate a therapeutic regimen which includes medication orders and reconciliation
  • Conduct physical exams of the patient during admission and attend to basic medical needs/issues; work with hospital specialists to manage more complicated medical conditions
  • Perform admission work-ups within twenty-four (24) hours of admissions and provide appropriate documentation as may be defined by Hospital policies
  • Perform rounds and record patient progress notes; compile detailed narrative and case summaries; complete forms pertinent to patients’ medical records
  • Serve as a resource and role model for clinical staff
  • Ensure optimal patient care
  • Perform as part of the treatment team in planning, coordinating and implementing nursing care activities for patients or referrals of patients to the appropriate healthcare facilities and/or agencies, and other resources of the community or physician
  • Order laboratory, radiological and diagnostic studies appropriate for complaint, age, race, sex and physical condition of the patients
  • Assist Program leadership in assessing, creating and implementing advanced practice provider procedures, or ensuring existing procedures meet federal and state regulations and guidelines for Nurse Practitioner scope of practice
  • Follow voluntary and involuntary procedure/process
  • Interpret the clinical needs of the patient to nonclinical staff assigned to the patient
  • Provide telemedicine on-call coverage utilizing tele psychiatry and patient round psychiatric services for Client’s psychiatric program. Utilize tele psychiatry and patient round tele psych
  • Provide on-call services on a twenty-four (24) hour basis at such times as set forth on a service schedule (as needed)
  • Insure patient dignity and respect patient values
  • Establish a compassionate environment by providing emotional and psychological support to patients, families and friends
  • Maintain patient privacy and confidentiality; and protects operations by keeping patient and program information confidential
  • Operate within ethical standards
  • Collaborate with physicians, multidisciplinary team members, and outside health and social agencies.
  • Complete all required documentation relevant to patients in a timely, efficient manner.
  • Adhere to Hospital’s Medical Staff Bylaws, Rules & Regulations and Policies & Procedures
  • Assure quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and regulatory standards
  • As needed, and using approved methods, assist in safely placing patients in seclusion or restraints; observe suicide/seclusion precautions; including physical requirements for physical holds and safe transport of patients
  • Maintain continuity among the clinical team by documenting and communicating actions, irregularities, and continuing needs
  • Protect patients and employees by adhering to safety standards; infection-control policies and protocols; medication administration and storage procedures; and controlled substance regulations
  • Use hygienic practices and teach patient hygienic practices
  • Complete all required staff competencies per program regulations in a timely manner
  • Renew licenses and certifications in a timely manner
  • Plan and implement nursing groups if required
  • Ensure operations of equipment by alerting designated department or administrative person is aware of any malfunctions
  • Provide feedback to supervisor on ongoing basis in regard to concerns, improvements, changes, etc.
  • Audit charts
  • Orient or mentor new clinical staff as needed
  • Other duties as assigned

Qualifications

  • Master's Degree in Nursing accredited through the CCNE or ACEN
  • Active State RN license
  • Doctor of nursing practice (DNP) highly preferred
  • 3 years of clinical experience as an Advanced Practice Provider in psych preferred
  • Previous experience in behavioral health highly preferred
  • CPR Certification
