What are the responsibilities and job description for the Patient Navigator position at UHS?
The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. They will also meet with patients discharging to the community to understand their patient experience. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. The step-down Navigator will also assist patients in understanding the court process and work with the court systems to ensure strong communication and collaboration.

Qualifications
Education: Bachelor’s Degree in mental health/behavioral sciences field.
Experience: Minimum two (2) years’ experience working in psychiatric/mental health field
