Behavioral Health Connections (BHC) in Atlanta s seeking a dynamic and talented Referral Managment PRN to join the team of compassionate, dedicated professionals. The Referral Managment PRN serves to assess and coordinate he care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs.
Job Duties/Responsibilities:
- Perform all aspects of the hospital assessment and referral process.
- Service coordination by matching patients with appropriate mental health services
- Providing and assisting patients with resource navigation
- Faciliatate the scheduling of the intial intake and ensuring smooth transition to care

Qualifications
Requirements:
Masters degree in Counseling, Psychology, Social Work or Social Services field preferred.
Preferred License in the State of Georgia as a Licensed Professional Counselor (“LPC”), Licensed Master Social Worker (“LMSW”), Licensed Clinical Social Worker (“LCSW”)
