Position Summary
The Pharmacist provides comprehensive clinical pharmacy services to home infusion patients at University of Iowa Community HomeCare, ensuring compliance with all applicable laws, regulations, and accreditation standards. This role is responsible for managing controlled substances, developing and implementing individualized care plans, monitoring and optimizing medication therapy, reviewing laboratory results, and collaborating with healthcare providers to ensure safe and effective treatment. The Pharmacist also plays a critical role in educating patients and caregivers, documenting clinical activities, monitoring adverse drug reactions, and maintaining seamless coordination of home infusion therapy. Additionally, the Pharmacist serves as a mentor and leader, coaching Pharmacy Technicians, Interns, and other staff to foster a culture of excellence and continuous learning.
Position Responsibilities
- Accurately enter home infusion orders into the electronic health records (EHR) system, ensuring appropriate drug selection, dosage, administration route, and compatibility based on drug stability, patient needs, and clinical best practices.
- Review, verify, and approve compounded and prepared sterile infusion products for accuracy, sterility, compliance with USP
guidelines, and adherence to safety protocols before dispensing.
