Position Summary
The Clinical Faculty (Open rank) / Clinical Pharmacist serves on an interdisciplinary care team and/or as a primary health care provider responsible for assuring and promoting the safe, rational, efficacious, and cost-effective use of drug therapy with the ultimate goal of achieving positive clinical outcomes and improving a patient's quality of life. To serve as an educator of pharmacy students, pharmacy residents, international scholars, and other members of the healthcare team through providing lectures and inservices, and precepting experiential education rotations. To facilitate the advancement of knowledge in health care by participating in research and scholarly activities.
Duties & Responsibilities
• Patient Care – Non distribution:
• Attend and actively participate in service and/or attending rounds or other service related functions.
• Perform those duties necessary to accurately and completely monitor therapeutic regimens: i. Review assigned patient's clinical notes, laboratory summaries, medication administration records, problem list, and medication list on each patient. ii. identify and monitor those objective and subjective parameters appropriate to identify efficacious drug therapy or evidence of adverse effects. iii. Continually reevaluate pertinent parameters and adjust patient’s therapeutic regimens as necessary
• Perform those duties necessary to facilitate therapeutic decisions including the location, retrieval, interpretation, summarization, and dissemination of current literature concerning a patient's drug therapy.
• Conduct and document medication histories on selected/appropriate patients.
• Intervene whenever necessary to improve the quality of patient care, provide drug information and ensure rational, safe, and cost effective drug therapy.
• Assure continuity of pharmaceutical care by providing pharmacists, physicians, nurses and other healthcare professionals with necessary information related to a patient's therapeutic regimens.
• Participate in departmental cost containment programs by aggressively identifying and intervening on high cost/target drugs.
• Provide patients and their families/caretakers with information related to the proper use of discharge medications in order to assure safe and effective use of, and compliance with, prescribed medications.
• Evaluate all non-formulary drug requests originating in the assigned patient care area; document approval or suggestion of formulary alternatives as appropriate.
• Provides pharmacokinetic dosing assessment and recommendations for appropriate drugs or in appropriate patients, and documents in patient’s electronic health record (EHR)
• Document in the EHR or other appropriate place, patient care activities including but not limited to: pharmacokinetic consultations, medication teaching notes, medication histories, pharmacotherapeutic interventions, adverse drug reactions and other significant recommendations or activities.
• Patient Care Distribution: Assure that drug delivery and administration conform to departmental policy and procedure and ensures maximal patient safety.
• Assist in providing accurate, efficient and appropriate drug distribution services when needed.
• Facilitate communication and cooperation between all individuals involved in the drug use process to assure the accurate and timely provision of drug products.
• Respond and participate in CPR, RRT, and stroke emergencies by carrying the Adult CPR, Peds CPR and RRT pager as assigned
• Patient Care -Population Based: Review, evaluate and help revise Hospital and Departmental policy and procedures as appropriate; and assure that such policy and procedures are being met.
• Actively participate in Departmental quality assurance programs, including: reporting of adverse drug reactions and medication errors.
• Work with the Senior Associate Director of Clinical Services and the Director, Clinical Services.
