Position Summary
Recommend, develop, and implement Home Care Program activities related to policy/procedure interpretation and assurance of professional quality.
Duties & Responsibilities
Reviews Home Care applications and Medical Plans for Home Care prepared by regional coordinators for compliance with the home care policy and procedures and state and federal regulations. Completes Levels of Care on waiver participants.
Performs quality assurance activities that assure the approved nursing agencies are in compliance with applicable State standards and DSCC regulations.
Implements the review process and documentation used to audit service providers throughout the State of Illinois which includes audits at service provider sites, following up with letters explaining the findings, requesting changes that ensure the program’s policies and procedures are being followed.
Conducts participant record reviews at regional office level to verify compliance with program requirements; advises the Regional Managers, the Home Care Quality Improvement Manager, the Assistant Directors for Operations, and the Associate Director of Care Coordination, of compliance with the Home Care Program policy and procedures
Provides consultation and policy interpretation to the Regional Office coordinators including assistance with the eligibility process, care coordination activities, termination of eligibility, critical/non-critical incidents, and interaction with involved providers of home care services.
Identifies Home Care Program staff development needs of regional coordinators in conjunction with the Regional Managers. Makes appropriate recommendations and/or provides training.
Manages special projects related to the Home Care Program as directed by the Home Care Quality Improvement Manager and the Associate Director of Quality Improvement and Education.
Attends meetings and conferences as assigned by the Home Care Quality Improvement Manager or the Associate Director of Quality Improvement and Education
Performs other duties as assigned.
