Nurse Practitioner Job Description
Working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of skilled nursing facility residents. The APRN will establish and build strong working relationships with administrators and staff throughout their designated building and will act as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what you love - working independently to see patients and treat complex medical conditions. Nurse Practitioner Responsibilities
Provide primary, urgent and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents.
Verify admission orders and conduct initial assessment of residents. Conduct Physical exams, medication review, and depression screening
Collaborate with facility medical director, attending physicians and nursing staff to deliver the highest level of care to the patient
Communicate with and counsel family members on treatment options when there is material changes in status
Minimum Qualifications
Valid Nurse Practitioner License in Kentucky
DEA License
Nurse Practitioner national certification
A clinical background in adult, family or geriatric specialties
Vaccination against COVID-19 is required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.
Preferred Qualifications
Experience in internal medicine, gerontology, or cardiology
Excellent administrative, organizational and verbal skills
Ability to work independently
Detail oriented
Dependable and reliable
Nursing experience in LTC / SNF a plus'
