About the Role
As a Nurse Practitioner (NP) with Ultra Health Providers, you will play a vital role in managing the medical needs of skilled nursing facility residents. You will work closely with facility staff, collaborate with attending physicians, and serve as a primary point of contact for urgent care needs. Our NPs are provided with the training, tools, and autonomy to deliver high-quality care independently while managing complex medical conditions.
This position includes a shared on-call rotation to ensure continuity of care for residents. On-call responsibilities are thoughtfully scheduled among the team to maintain work-life balance.
What You’ll Do
Provide primary, urgent, and preventive care to residents in assigned skilled nursing and assisted living facilities.
Perform admission assessments, conduct physical exams, review medications, and screen for depression.
Work in partnership with the facility Medical Director, attending physicians, and nursing staff to ensure the best possible care for residents.
Communicate with and support family members regarding treatment plans and changes in patient status.

Nurse Practitioner Minimum Qualifications
- Valid Nurse Practitioner License in Georgia
- DEA License
- Nurse Practitioner national certification
- A clinical background in adult, family or geriatric specialties
- Vaccination against COVID-19 is required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.
