MDS Coordinator / Care Plan Coordinator
About the Role:
We are seeking a dedicated MDS Coordinator / Care Plan Coordinator to lead the coordination of accurate resident assessments and personalized care plans that align with Federal and State regulations. This position plays a vital role in evaluating resident needs, determining the appropriate level of care, and collaborating with families and team members to ensure the highest quality support.
Key Responsibilities:
- Oversee and integrate nursing, dietary, therapy, recreation, and medical services to create accurate assessments and optimize reimbursement.
- Complete all Medicare/Medicaid case-mix documentation to support proper reimbursement for services rendered.
- Conduct ongoing reviews of Minimum Data Set (MDS) to achieve the best possible Resource Utilization Group (RUG) classifications.
- Manage the end-to-end process of MDS/Prospective Payment System (PPS) documentation, submission, and tracking.

Preferred Qualifications:
- Current and unencumbered Registered Nurse (RN) licensure.
- Prior experience in long-term care settings is highly desirable.
