I. Major Responsibilities:
Maintains the scheduling needs of the nursing and rehabilitation teams by preparing patient schedules, ensuring visits are assigned to appropriate staff in accordance with clinical competencies/skills set, tracking/reviewing staff daily schedules, coordinating schedules to minimize the use of overtime, and minimizing the number of clinicians scheduled to visit patients. Notifies appropriate manager of under/over scheduling for individuals or missed visits. Coordinates scheduling of clinical experiences for clinicians according to the orientation/training plan established by the staff educator/management team. Collaborates with the weekend coordinator to provide effective and efficient scheduling for staff on weekends and holidays. Obtains and processes physician orders for initiation and resumption of services according to standards of practice and departmental policy and procedures. Performs intake responsibilities which includes triaging clinical issues within the scope of responsibilities as an LPN. Refers clinical issues outside scope of responsibilities to the appropriate clinical staff. Performs occasional home visits within the scope of responsibilities as an LPN. Accepts, researches, and processes referrals according to departmental procedures and policies. Refers unusual referral requests to the management team or designee according to departmental policy. Ensures all pertinent clinical information is obtained and documented in the electronic medical record.

II. Position Qualifications:
License/Certification/Education:
Required:
1. Completion of an accredited practical nursing program.
2. Current Massachusetts LPN licensure.
3. Valid and unrestricted driver's license and access to a reliable, insured vehicle.
Preferred:

Experience/Skills:
Required:
1. Minimum two (2) years of general health care experience.
2. Excellent oral and written communication skills.
3. Excellent organizational skills and attention to detail.
4. Knowledge of basic nursing procedures.
Preferred:
1. Minimum one (1) year of clinical experience in a certified home health agency.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Work is considered sedentary. Position requires work indoors in a normal office environment.
