Job Details
1-year assignment.
BSN and 2-years of experience with Neurology.
This position entails following a patient that has had a stroke or seizure 90 days post discharge.
Follow up with the patient if they go to a SNF / LTAC make specialist or PCP follow up appointments medication management home healthcare and will assist with getting the patient to a clinic if there are noted concerns.
Homecare or case management experience preferred.
M-F 40 HPW.

Additional Details
Valid ID is required.
TIME SENSITIVE : Required for all candidates and updated annually.
