Job Description
Performs medical and psychosocial evaluation of patient with chronic conditions, end of life and comfort care; extensive involvement in discharge planning and continuity of care efforts; and support and collaboration with primary medical and nursing staff responsible for the patient. Acts as a member of an interdisciplinary professional team providing pain management consultation for patients. All work is performed in close collaboration with primary physicians, house staff, nursing staff and social services.
Qualifications
Education & Training : Nurse Practitioner with appropriate Maryland licensure and credentialing to practice under the supervision of a physician in the state of Maryland. ANCC or Palliative & Hospice Medicine certification is preferred.
Work Orientation & Experience : A minimum of five (5) years of direct patient care experience in internal medicine or palliative care, hospice. Experience is appropriate to the management of chronic disease, end of life, comfort care and goals of care.
