As a dedicated advocate, you will ensure access to vital services, facilitate referrals, and educate members on managing chronic conditions and preventive care strategies.
Your Impact
- Conduct comprehensive assessments in various settings—home visits, telehealth, and community-based locations—to develop personalized care plans tailored to members' needs and goals.
- Serve as an advocate to ensure members access appropriate services and resources. Provide referrals to specialists, diagnostic services, and community support programs as needed.
- Utilize clinical knowledge and evidence-based practices to educate members about chronic condition management, medication adherence, and preventive care strategies. Collaboratively monitor member progress and adjust care plans as necessary.
- Collaborate with multidisciplinary healthcare teams to coordinate holistic care plans.
- Maintain precise documentation of interactions, care plans, and coordination activities in compliance with policies and regulations.
- Participate in community outreach and health education initiatives to promote health literacy and preventive healthcare.
- Regularly evaluate care plan effectiveness and adapt based on member needs.
- Facilitate seamless transitions between healthcare settings or levels of care for members, ensuring continuity and adherence to best practices.
- Provide guidance for navigating complex healthcare systems, including insurance coverage and scheduling specialty services.
- Perform additional duties as assigned to support the organization's vision, mission, and values.
Your Credentials
- Must be licensed as RN, LCSW, OT, RT, certified CRC, CDMS, or other related certifications.
- Bachelor’s degree in nursing preferred
- Ability to assess patients and diagnose symptoms.
- Minimum of 2 years’ experience in case management, care coordination, or similar roles.
- Proficiency in developing, implementing, and evaluating individualized care plans.
- Excellent verbal and written communication skills for effective collaboration with patients, families, and healthcare team members.
- Strong analytical skills for problem-solving and addressing patient care issues.
- Capacity to foster positive relationships with members and healthcare professionals.
- Exceptional organizational and time management skills with task prioritization abilities.
- Experience with electronic health records (EHR) and healthcare management software.
Compensation Salary varies based on experience, education, and location: $60,000–$70,000 annually.
