Your Impact :
- Serve as the primary point of contact forpatients, caregivers, and providers, helping facilitate seamless transitions ofcare and addressing any barriers along the way.
- Conduct initial assessments to understandpatient needs, goals, and any obstacles they may face in accessing care.
- Develop personalized care plans incollaboration with healthcare teams, tailoring support to meet each patient'sunique needs.
- Coordinate appointments, referrals, andtests to ensure patients have timely access to the care they require.
- Advocate for patients to make sure theyreceive necessary treatments, medications, and resources to support theirhealth journey.
- Provide education to patients andcaregivers on health conditions, treatment plans, and effective self-managementpractices.
- Track patient progress and outcomes,adjusting care plans as needed to promote the best possible health results.
- Accurately document patient interactions,care plans, and interventions in electronic health record (EHR) systems.
- Work with insurance providers and connectpatients to community resources to address social determinants of health andensure access to necessary supportive services.
- Participate in care conferences and teammeetings to discuss patient goals, progress, and discharge planning.
- Perform other duties as assigned bymanagement needed to help drive to our Vision, fulfill our Mission, and abideby our Organizations Values.
Your Credentials :
Experience with insurance coordination andknowledge of social determinants of health is a plus, but not required forentry-level applicants.
