The Role :
The Program Manager, Population Health plays a critical role in managing and enhancingUmpqua Healths Population Health Program. This position is responsible for thedevelopment, implementation, and oversight of initiatives aimed at improvingpopulation health outcomes, reducing health disparities, and increasingpreventive care engagement. By leveraging data-driven insights, the PopulationHealth Program Manager ensures the effective execution of population healthstrategies, including quality metric improvement initiatives and member outreachefforts. The primary focus of this role is to enhance the health and well-beingof members by implementing population-based interventions, facilitatingpreventive care efforts, and fostering cross-sector collaboration withhealthcare providers and community organizations.
Your Impact :
- Lead the development, implementation, and evaluation of Umpqua Healths Population Health Program to drive improved health outcomes and reduce health disparities.
- Collaborate with internal and external stakeholders, including providers, community organizations, and public health agencies, to develop targeted strategies for addressing social determinants of health.
- Utilize data analytics to identify high-risk populations, track health trends, and implement data-informed interventions that enhance population health outcomes. Develop and oversee outreach initiatives that promote preventive screenings, immunizations, chronic disease management programs, and other key health interventions.
- Conduct timely outreach and member engagement efforts to close care gaps, improve quality metric performance, and promote health education initiatives.
- Support value-based care initiatives by ensuring alignment with key performance metrics and quality improvement goals.
- Design and implement culturally and linguistically appropriate health education and outreach materials to improve member engagement and health literacy.
- Monitor and report on program effectiveness, including key performance indicators and outcome measures, to ensure continuous improvement.
- Work collaboratively with internal teams, including the Quality and Care Management teams, to support evidence-based decision-making and care coordination efforts.
- Advocate for policies and initiatives that support population health improvement efforts, including access to healthcare services, preventive care, and chronic disease management resources.
- Ensure compliance with regulatory and accreditation standards related to population health programs.
- Perform other duties and support deliverables as assigned by the organization to help drive our Vision, fulfill our Mission, and abide by our Organizations Values.
