As the Program Manager of Population Health, you will lead initiatives designed to improve patient outcomes, manage chronic conditions, and promote preventive care. Your strategic oversight and ability to collaborate across teams will play a critical role in advancing Umpqua Health’s vision for a healthier community.
Key Responsibilities
- Develop, implement, and oversee population health programs aimed at addressing chronic diseases, social determinants of health, and preventive care strategies.
- Manage multiple projects simultaneously, ensuring alignment with organizational goals, regulatory requirements, and evidence-based practices.
- Collaborate with internal stakeholders, healthcare providers, and community organizations to coordinate population health initiatives and services.
- Utilize data and analytics to identify trends, track program performance, and implement targeted interventions to improve patient outcomes.
- Lead efforts to reduce health disparities by designing programs that address equity in access to care and resources.
- Monitor and evaluate the effectiveness of programs, leveraging insights to refine strategies and optimize outcomes.
- Manage budgets, resources, and operational workflows to ensure efficiency, sustainability, and program success.
- Build and maintain relationships with external partners, including public health agencies, insurers, and non-profit organizations, to expand the reach and impact of population health initiatives.
- Develop and deliver presentations or reports to leadership and stakeholders on program progress, outcomes, and recommendations.
- Perform other duties as assigned to support Umpqua Health’s Vision, Mission, and Values.
What You Bring
- A Bachelor’s or Master’s degree in Public Health, Healthcare Administration, or a related field (required).
- 5 years of experience in population health, public health, or healthcare program management, with a strong understanding of community health initiatives.
- Proven ability to design and implement programs that improve health outcomes and address health disparities.
- Experience working with data analytics tools and platforms to track and report on program effectiveness.
- Strong project management skills, with the ability to manage multiple initiatives and balance competing priorities.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders.
- Knowledge of social determinants of health, value-based care models, and public health policies.
- Budget management and resource allocation experience in a healthcare or public health setting.
- Passion for improving health equity and addressing disparities in healthcare delivery.
Rate varies on experience, education, location $65,000-$75,000
