Job Title: Care Coordination Program Director


Job Status: Full time, exempt, grant-funded


Job Location: Remote for now


Job Summary: Unbound Now, with the endorsement of regional advisory councils and the financial support and direction of the Office of the Governor’s Child Sex Trafficking Team, is committed to implementing the Texas Model for Care Coordination for Commercially Sexually Exploited Youth (CSEY). Care coordination facilitated by Unbound Now will be consensus-driven, collaborative, and driven to identify and recover CSEY and to facilitate tailored, accessible, trauma-informed, and holistic resources through a coordinated network of providers. The goal is for every identified youth survivor of sex trafficking to have access to non-punitive, responsive, high-quality, community-based services that meet their unique short-term and longer-term needs. Care coordination includes awareness, education, creativity, collaboration, continuous learning, and capacity-building to identify and recover CSEY youth. Care coordination teams build trust, transparency, and solutions with each other to mitigate duplication of work and so that local and statewide partners are bridges instead of barriers to services for youth and their families.


The primary functions of the Care Coordination Program Director are to maintain and continue to build Unbound Now’s care coordination program as described above, providing leadership and oversight for care coordination across locations, supervising regional care coordination staff to ensure regional consensus-building and protocol development, compliance with Unbound Now policies and procedures and the expectations of the Texas Office of the Governor Child Sex Trafficking Team, effective training for care coordination staff, and excellence in service delivery; develop and maintain strong relationships with statewide partners; ensure timely and accurate programmatic reporting for grants and other purposes; serve as the administrator for the care coordination program on Unbound Now’s case management system; present on Unbound Now’s care coordination program and services as needed; and support development of care coordination services in new areas.

 

Compensation: Annual salary


Benefits: Option of health insurance for employee and family, option to participate in Unbound Now’s retirement plan (3% salary match after 90 days with 1-year vesting period), and life insurance for employee


Availability:  Generally, Monday through Friday 8:30-5:30. Weekends, evenings, and holidays as needed. Anticipated 45-50 hour work week. Travel to Austin and Unbound Now care coordination service regions will be needed.

 

Working Conditions: Work performed in a normal office environment or other professional environment. 


Job Responsibilities: 

  • Maintain and continue to build Unbound Now’s care coordination program in accordance with the Texas Office of the Governor Child Sex Trafficking Team’s expectations as set forth in The Texas Model for Care Coordination Grant Program, FY2024-25 funding announcement and any subsequent direction provided by the CSTT
  • Provide leadership, support, and supervision to Unbound Now’s care coordination teams
  • Recruit, screen, train, engage, supervise, and retain new care coordination staff
  • Ensure development of regional protocol, supporting as needed
  • Ensure 24/7 care coordination in all service regions
  • Encourage engagement of CSEY advocacy services
  • Ensure that regional care coordination teams facilitate rapid response meetings and service staffing meetings
  • Ensure regular meetings of advisory councils in each service region
  • Ensure that confidential information is released only with parent/guardian consent
  • Ensure that regional care coordination teams are promoting and hosting awareness events and education/training events
  • Ensure that regional care coordination teams are conducting case analyses
  • Ensure that regional care coordination teams are conducting data evaluation sessions
  • Maintain a strong line of communication with the COO regarding any issues related to Unbound Now’s Care Coordination Program
  • Facilitate and lead regular gatherings of all Unbound Now care coordination staff to ensure consistency in service delivery and adherence to policies and protocol
  • Present on Unbound Now’s care coordination services as requested
  • Contribute to any continued build-out needed of Unbound Now’s care coordination program on its case management system, ensuring an appropriate means of data collection for all reporting requirements
  • Serve as the administrator for the care coordination program on Unbound Now’s case management system, adding and subtracting care coordination users as needed, ensuring that needed updates are made in a timely manner and reports continue to run smoothly and are updated as needed based on changes to the software or funding source requirements
  • Develop and maintain good working relationships with essential statewide partners, such as the Texas Office of the Governor’s Child Sex Trafficking Team and DFPS
  • Attend and participate in statewide calls related to care coordination
  • Support the development of protocols and care coordination services in new areas
  • Ensure that all required training for care coordination staff is completed and documented in a timely manner 
  • Develop Unbound Now care coordination program budget and ensure adherence to budget, requesting adjustments as needed
  • Support regional care coordination staff in seeking and reviewing feedback from regional partners and provide coaching and support based on feedback results
  • Seek and review feedback from statewide partners and coach regional care coordination staff in making improvements based on feedback received
  • Review all care coordination program incidents and responses, ensuring appropriate response and documentation
  • Review all care coordination program grievance reports, ensuring appropriate response and documentation
  • Oversee abuse prevention and response efforts to mitigate risk of abuse within Unbound Now’s care coordination program and ensure proper reporting
  • Support regional care coordination staff in responding appropriately to allegations of abuse, including youth-to-youth sexual activity
    • Take allegations seriously
    • Respond immediately - if there is a reason to believe behavior is abusive, report to authorities
    • Report to COO immediately
    • Collect information, as appropriate 
    • Develop and document the response and corrective action
    • Determine and meet the needs of the alleged victim, as appropriate
    • Prevent further access by alleged perpetrator
  • Collect, review, and report programmatic data statewide, assessing program effectiveness and ensuring efforts are made to measure performance and impact and improve where needed
  • Incorporate lived experience input from survivors into care coordination program evaluation and development
  • Ensure resilience-building and self care and wellness training are available for all care coordination staff to support longevity in their roles and healthiness in their interactions with one another and the survivors, families, and partner agencies they serve
  • With input of regional care coordination staff, set goals and provide data for care coordination-related grant applications
  • Ensure that all needed written materials describing Unbound Now’s care coordination services are accurate, consistent, and available in each service region

  • Accurate and timely programmatic reports
  • Communities and partner agencies understand Unbound Now’s care coordination services, with strong public presentations and written materials available as needed
  • Care coordination built out in new regions as opportunities arise

Experience and Education:

  • Advanced, relevant degree preferred
  • Leadership and supervisory experience 
  • Experience working with youth who have experienced commercial sexual exploitation
  • Experience working collaboratively with statewide and regional partner agencies
  • Comfortability developing and facilitating trainings
  • Experience with programmatic grant reporting
  • Experience using cloud-based case management software
  • Experience facilitating protocol development
  • Trained and experienced in trauma-informed care
  • Trained in use and supervision of motivational interviewing

Job Requirements:

  • Mature Christian faith reflected in regular attendance at a local Christian church
  • Provide at least three references (supervisor, other professional, and personal)
  • Thorough commitment to the vision, mission, and values of Unbound Now
  • Agree to and pass all required criminal background checks, including DFPS criminal history check and abuse and neglect registry check
  • Pass employment authorization verification
  • Fulfill Unbound Now's training requirements
  • Ability to build and maintain consensus
  • Excellent organizational and administrative abilities
  • Excellent communication, interpersonal, collaborative leadership, and supervision skills
  • Strong public presentation skills, in person and online
  • Culturally competent
  • Ability and willingness to maintain confidentiality of sensitive information
  • Ability to problem solve and provide coaching as needed
  • Abide by Unbound Now policies at all times
  • Willingness to travel among Unbound Now service regions using personal vehicle, reliable vehicle, valid driver’s license, and car insurance
