The Case Manager is a skilled position. The Case Manager is a hybrid position that fills the needs to make collateral contact and coordination of care with outside agencies on behalf of our clients. These duties include contacting probationary officers, court personnel, Child protective services, Department of Economic Security, and other agencies as required. This position also augments the daily duties of a BHT in the milieu. More specifically, the facilitation of group classes plays an essential role in the Case Manager. The Case Manager will work closely with the counselors and assist them with their caseload. This position is filled both through internal and external hiring. This position operates in an inpatient/residential environment, or an outreach environment, such as community agencies. This position assists in progressive programmatic development using an integrative model of care with consideration of both behavioral and physical health issues. Provides screening, assessment, treatment planning, life skills, wellness and prevention activities, pre-vocational training, crisis intervention, individual or group counseling services to residents and clients. Plays a significant role in providing input, treatment, and coordination for physical health, wellness, and prevention as a member of a clinical team on site. Contributes to a recovery-based environment.

Duties and Responsibilities

1. Provides direct services to patients in a manner that promotes safety, comfort, and the therapeutic environment.

2. Provides input to the treatment process.

3. Assists patients with daily living attend to patient behavioral problems, and assists in crisis intervention.

4. Provides prevention and intervention that help patients maintain their recovery.

5. Attends and participates in in-service training sessions and related training opportunities as made available.

6. Participates in the development and implementation of patient treatment programs.

7. Maintains confidentiality of patient records.

8. Transport patients to and from appointments, clinics, laboratories, and/or treatments, as and when appropriate.

9. Attends and participates in group sessions, in-service education, and staff meetings.

10. May need to cover evening/over-night/weekend hours as needed.

11. Makes contact with external facilities and agencies to promote continuity of care.

12. Performs miscellaneous job-related duties as assigned.

13. Conducts patient UA's as appropriate.

14. May assist in administering medication to include narcotics to patients in Medicated Assisted Treatment (MAT) when appropriate.

15. Planning, implementing, evaluating, and monitoring treatment activities and groups

16. Provide transportation for patients to court, medical, and other appointments as deemed appropriate by the supervisor.

17. Verify enrollment in AHCCCS and support enrollment of the uninsured.

18. Serve as a liaison inpatient warrant verification.

19. Coordinate with medical personnel for any matter involving medical appointments to include ensuring follow up appointments and transportation are established.

20. Provide access to DES and social welfare programs to promote and maintain positive health outcomes upon discharge.

21. Serve as a backup for intakes and discharges when necessary.

22. May conduct hourly accountability/security walks.

23. May conduct daily house meetings.

24. Other duties as assigned by the supervisor.

Knowledge, Skills, and Abilities Required

· Knowledge of crisis intervention and de-escalation techniques to prevent violent or harmful behavior to self or others.

· Ability to interact with patients, faculty, and/or staff in a team environment.

· Ability to maintain quality, safety, and/or infection control standards.

· Ability to observe, assess, and record symptoms, reactions, and progress by obtaining vital signs such as blood pressure, pulse, and temperature using manual and electronic devices.

· Ability to provide therapeutic patient care to mentally, emotionally, and/or developmentally

· Knowledge of RBHA and T/RBHA as well as AHCCCS requirements.

· Skill in preparing and maintaining patient records.

· Ability to communicate and interact with mental health professionals in a team environment.

· Ability to make administrative and procedural decisions and judgments on sensitive, confidential issues.

· Knowledge of basic EHR entries includes documentation of group classes and other miscellaneous case management entries, both billable and non-billable.

· Maintain appropriate boundaries with patients.

· Provide direct patient care, guidance, and positive role-modeling

· Knowledge of community resources involving housing, mental health assessment, food vouchers, and various social welfare programs.

· Working knowledge of the judicial system and pretrial services.

· Bilingual Preferred

Conditions of Employment

· Must be 21 years of age or older.

· CPR/ 1st Aid Certified

· The successful candidate must submit to a TB test and Urine analysis.

· Must possess a level one fingerprint clearance card within 90 days of employment through the Department of Public Safety.

· If in recovery, must have a minimum of 1-year sobriety before application.

· Drivers must have a clean 39-month vehicle record check

Working Conditions and Physical Effort

· Moderate physical activity. Requires handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.

· Moves intermittently during working hours.

· Works in office areas as well as throughout the facility and its premises.

· It involves residents, family members, personnel, visitors, government agencies, and personnel all under various conditions and circumstances.

· Is subject to exposure to infectious waste, disease, conditions including TB and AIDS, and Hepatitis viruses.

· The work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include protective equipment.

· Work involves moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.

Education and Experience

Any of these will meet the requirements for the position:

-High school diploma/GED and four years' experience in behavioral health and one year experience in case management or associate degree and two years' experience in behavioral health and one year experience in case management or Bachelor's degree and one-year experience in behavioral health or case management.

· Attends and participates in continuing education programs.

· Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants

· Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses

· Must be able to read, write, speak, and understand the English language

· Must possess the ability to make independent decisions when circumstances warrant such action.

· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

· Must possess leadership ability and willingness to work harmoniously with other personnel.

· Must not pose a direct threat to the health and safety of other individuals in the workplace.

· Must function independently, have flexibility, personal integrity, and work effectively with the residents, personnel, and support agencies.

· Must meet the general health requirements set forth by policies of this facility, which include a medical and physical examination.

· Must be able to cope with the mental and emotional stress of the position.

· Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.

· Must be able to push, pull, move, and/or life a minimum of 25 pounds to a minimum height of five feet and push, pull, move, and/or carry such weight a minimum distance of 50 feet.

· May be subject to the handling of and exposure to hazardous chemicals

· It may be necessary to assist in the evacuation of residents during emergencies.
