JOB DESCRIPTION SUMMARY
Responsible for planning, coordinating, and managing all volunteer program activities in the assigned region.
VOLUNTEER COORDINATOR JOB FUNCTIONS/RESPONSIBILITIES
1. Develop and maintain volunteer training and orientation program sufficient to meet the volunteer needs of the patients and families.
2. Recruits, educates, and selects volunteers from at least three recruitment sessions a year.
3. Establish a need for volunteer services with the patient and family/caregiver and entire hospice team.
4. Supervise and evaluate volunteers in a timely and appropriate manner.
5. Maintains monthly statistics. Records services of volunteers, including the type of service performed and the time worked.
6. Coordinate support/education meetings.
7. Attends interdisciplinary group meetings.
8. Provides information to individuals and community groups about hospice volunteer services.
9. Actively participates in quality assessment performance improvement teams and activities.
10. Participates in providing, reviewing and revising written volunteer policies and procedures.
11. Accepts and performs other related duties and responsibilities.
INTAKE COORDINATOR JOB FUNCTIONS/RESPONSIBILITIES
1. Receives and processes referrals immediately, prioritizing these over other duties.
2. Processes new patient referral information into the computer system in a timely manner.
3. Verify insurance and requests authorization if needed.
4. Establishes and maintains positive relationships with current and potential referral sources.
5. Performs clerical duties such as, but not limited to, data entry, faxing, scanning and copying.
6. Assists with electronic and hard copy medical record including admission, discharge and audits.
7. Review Eligibility Alerts and Responses for Medicare and Medicaid Clients
8. Manages pending referrals daily and consults with ED on patients in pending status.
9. Answers the telephone, takes and delivers messages by phone, verbally, or in writing.
10. Supports agency with weekend and holiday on call, securing coverage in advance if a schedule change is needed.
11. Safeguards confidentiality of patient information per HIPAA regulations.
12. Must adhere to the company attendance and reliability guidelines, which are an essential function of the job to ensure quality, and consistent patient care.
