S UMMARY :

Under the direction of the Support Services Director and Manager, the Service Consultant assists the individual and their families / guardians to explore and gain services and supports from a variety of sources. Assures that services delivered adhere to all regulatory requirements, as well as UCBDD policy, regulations and procedures. The Service Consultant must strive to meet the UCBDD published Mission Statement and exemplify the UCBDD defined Core Values.

QUALIFICATIONS

QUALIFICATIONS :

Education / Experience -

  • Bachelor's Degree in Human Services, Business Administration, or related field.

Credential / License -

  • Must possess or meet all of the DODD requirements to possess and maintain Service and Support Administrator Certification under OAC 5123 : 2-5-02.
  • Must obtain and maintain First Aid and CPR certification.
  • Other -

  • Must have reliable transportation.
  • Must maintain a valid State of Ohio Driver's License if operating a personal vehicle for official agency business.
  • ESSENTIAL FUNCTIONS

    PRINCIPAL ACCOUNTABILITIES :

    Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to :

  • As assigned, coordinates the process for eligibility determination. Assesses and determines an individual's eligibility for county board services. Assists in gathering necessary documentation and creates / maintains all records relating to intake and eligibility. Tracks referral information, analyzes data, creates reports, and makes recommendations to the Support Services Director as required.
  • Maintains positive, collaborative relationships with individuals, families, team members and community partners.
  • Acts as the primary point of coordination for the effective development, implementation and coordination of the individual service plan (ISP).
  • Coordinates and completes assessments of the individual's needs, skills, risks, health and welfare initially and every 12 months.
  • Using person-centered planning, develops, reviews, and revises the individual service plan and ensure that the plan meets all requirements of OAC 5123-4-02.
  • Develops and authorizes a budget for services based on assessed needs and identifies appropriate funding sources.
  • Assists the individual with choosing providers while ensuring "Free Choice of Provider" per OAC 5123-9-11.
  • Provides ongoing coordination and monitoring in a variety of settings to ensure supports are provide in accordance with the individual service plan (ISP) and to the benefit and satisfaction of the person served. Ammends the ISP as needed to reflect changes.
  • Leads the individual's team and establishes and maintains contact with all team members, including the provider and natural supports, to ensure a clear understanding of the supports and expected outcomes of the individual service plan (ISP).
  • Completes all activities necessary for Home and Community-Based Waiver enrollment and redetermination.
  • Directs, facilitates and monitors employment activities as indicated in the individual service plan (ISP).
  • As assigned, and in collaboration with the transition team and school districts, develops, directs and implements a school-to-work transition program.
  • As assigned, coordinates OOD referrals for individuals exploring employment options.
  • As assigned, counsels individuals after job placement to ensure satisfactory work performance and suitability of job. Assesses the need for further services.
  • Maintains a working knowledge of benefits available through the Social Security Administration, Department of Jobs and Family Services and other resources, including work incentives.
  • Maintains knowledge of available resources; Links and refers individuals and families with community resources to meet needs.
  • Documents all Targeted Case Management (TCM) activities.
  • Understands informed consent and assists individuals and families with understanding and exercising their rights and responsibilities.
  • Advises individuals and families of their right to file an administrative complaint and / or request a hearing with the Bureau of State Hearings.
  • Reports Major Unusual Incidents and Unusual Incidents according to OAC 5123-17-02. Develops and implements prevention plans to reduce the likelihood of future occurrences.
  • Complies with all local, state, and federal rules, regulations, policies and procedures.
  • Participates in meetings, committees, trainings, and strategic plan initiatives as assigned.
  • Demonstrates regular and predictable attendance. Manages time and multiple tasks effectively.
  • Performs other duties as assigned.
  • SUCCESS FACTORS :

    To successfully perform this job, the following training, knowledge, skills and abilities are required.

    Required Training : All UCBDD employees shall be required to complete all general mandatory training. These include, but are not limited to :

  • Blood-borne pathogens, emergency communications, hazardous communications, slips / trips / falls, personal protective equipment, sexual harassment, drug free work place, violence in the workplace, MUI and fire prevention. HIPAA compliance.
  • Knowledge :

  • Proficient with computers and applicable software, ability to use general office equipment.
  • Working knowledge and understanding of behavior support, individualized service plans, medical, psychosocial, orthopedic and psychiatric aspect of disability.

    Working knowledge of client rights, federal, state, UCBDD programs and HIPAA. Knowledge of applicable statutes, regulatory standards and reporting authorities

    Language / Mathematical Skills :

  • Ability to communicate effectively, both orally and in writing.
  • Ability to perform basic math skills.

    Reasoning Ability :

  • Must be skilled in group facilitation, conflict resolution, crisis intervention and working through complex problem situations.
  • Other :

  • Must possess a positive customer service attitude and image to all stakeholder groups. Must be able to work collaboratively and effectively with other agencies, professionals, staff, families and the public.
  • Good leadership and organizational skills.

    Ability to keep accurate records.

    Must be able to tolerate the high demands of the position. The employee may be exposed to adverse weather conditions while driving and may be exposed to blood-borne pathogens, communicable diseases, potentially infectious materials and / or aggressive behavior.

    GENERAL EXPECTATIONS :

    Maintaining confidentiality is required. The employee is to report suspected or actual abuse / neglect, to follow the chain of command and to work effectively and cooperatively with others inside / outside the agency. Regular and predictable attendance is expected.

    Preserving a valid certification, licensure or registration is expected for those positions that require such in order to continue employment. It is essential for the employee to maintain all training and inservices required by the position.
