What are the responsibilities and job description for the Service Coordinator position at Union County Ohio?
Under the general supervision of the Supervisor, Deputy Director, and Director, provides casework and case management services to enable seniors to live independently in the community. Investigates/assesses elders alleged to be abused, neglected, or exploited; provides supportive services to elders in the least restrictive setting; receives and provides referrals to clients; develops and maintains relationships with providers and community partners; tracks and updates client information in the agency database; conducts home visits and performs routine periodic client assessments.

Bachelor’s degree in a Human-Services related field, or a combination of education and experience sufficient to perform the duties of the position.
LICENSURE OR CERTIFICATION REQUIREMENTS:
Must possess a valid Ohio driver’s license and maintain insurability under the County's vehicle insurance policy.

INHERENTLY HAZARDOUS OR PHYSICALLY DEMANDING WORKING CONDITIONS:
The employee has exposure to chemical compounds found in an office environment (e.g., toner, correction fluid, etc.); is exposed to possible injury due to unclean or unsanitary conditions; has contact with potentially violent or emotionally distraught persons; has exposure to hazardous driving conditions; has exposure to potentially vicious animals; has exposure to bed bugs, lice, scabies and other insects; works in or around crowds; ascends and/or descends ladders, stairs, or scaffolds; has to work irregular hours.
Note: In accordance with the U.S. Department of Labor physical demands strength ratings, this is considered sedentary work.(1) Conducts home visits with clients to perform assessments including personal and environmental safety, cognitive functioning, and coping skills; investigates Adult Protective Services (APS) cases including conducting face-to-face interviews with all parties of the case and collaterals formulates care plans from assessments and observations maintaining the elder in the least restrictive setting; completes required visits to families’ homes; completes referrals to law enforcement as necessary; completes all investigation/assessment activities following Federal, State and County policies and procedures and within mandated timeframes. Consistently and continually (e.g. at all times) represents the agency in a positive and professional manner and provides appropriate internal and external customer service.
(2) Documents all work related activities in the ODAPS and Wellsky systems within the established timeframes; Coordinates services with community partners; contacts clients periodically to assess needs and to ensure the level of service is appropriate, and quality is adequate; performs annual re-assessments; advocates for clients and educates clients regarding available services; completes additional reports as required such as Case Plans and Safety Assessments; completes County senior services enrollment forms (e.g. receipt of privacy practices, release of information, financial eligibility, etc.); attends and testifies in court hearings; participates in interdisciplinary team meetings and advocates for least restrictive and safe options for elders.
(3) Completes community outreach activities to educate the public on Senior Sales Tax Levy services. Participates on community boards that impact elder services.
(4) Attends staff meetings and training sessions as required.
(5) Maintains required licensures, certification, and continuing education requirements, if any.
(6) Meets all job safety requirements and all applicable safety standards that pertain to essential functions.
(7) Demonstrates regular and predictable attendance.
