Our growing orthopedic medical practice is looking for a qualified, team-oriented and energetic Physical Therapist to join our PT / OT / Rehab program.
We are looking for a skilled Physical Therapist who will be responsible for providing patient care including assessing, interpreting, planning and implementing specialized physical therapy treatments. Experience using TheraOffice is a plus.

SALARY : $100k- $120K ANNUALLY
QUALIFICATIONS :
Requirements : General :
Direct Patient Care : Evaluation and re-evaluation of patient
Assessment : Interprets evaluation data, history, and applicable medical documentation to determine rehabilitation potential and potential benefit from intervention; Identifies physical, social, and psychological needs of the patient, family, and significant other
Goal Setting : Utilizes patient / family / care giver input in setting functional rehabilitation goals; Documents goals in functional, measurable terms with stated time frames
Treatment Planning : Incorporates information gained in evaluation and assessment to determine appropriate, specific treatment plan for intervention and involves others when appropriate
Treatment Intervention : Demonstrates competence in performing all procedures and treatments required in area of practice demonstrates awareness of continuity of patient care by following established plans; modifies patient care based on continual assessment of the patient and effect of treatment intervention; demonstrates skills in identifying, interpreting, and incorporating patient's psychological, physical, and social responses to intervention; provides and / or recommends equipment or supplies for home use; and provides for patient's personal care and environment throughout the treatment
Patient Education : Provides education appropriate to each patient's level of understanding regarding evaluation, treatment, and self-responsibility and instructs in home treatment program to facilitate rehabilitation and promote independence in care as appropriate
Discharge Planning : Collaborates with patient, caregivers, and other health care professionals in planning discharge from rehabilitation services
Communication : Confers and communicates with referring physicians and other appropriate persons through the use of progress notes, letters, evaluations, team meetings, and direct contact concerning the proposed course, progress, and discharge plan of each patient

Pay : $100,000.00 - $120,000.00 per year
