Salary :

The Program Coordinator position is a full-time, non-exempt position. The Program Coordinator is responsible for Home and Community Services.

ESSENTIAL FUNCTIONS :

  • Planning / Scheduling : Lead the planning and coordination of the Home and Community Based Services.
  • Administrative Tasks : Support Administration in documentation, record retention and data conversion.
  • Case Management : Operate as a case manager for Home and Community Based Services recipients.
  • Supervisor Tasks : Oversee a team of PRN part time employees to deliver Home and Community Based Services inclusive of recruiting, training, documentation approval and payroll functions.
  • Customer Service : Provide high quality customer service, serving as a positive representative for the agency with other professionals, families, and the community at large.
  • Supervision : provide appropriate oversight to direct reports. Respond to individual safety needs, including reporting significant changes in behavior and health. Assist in addressing any undesired behaviors of individuals not covered in formal programs. Attend staff meetings and clinical supervisions, as required.
  • Additional Duties : All other duties as required by supervisor to meet the needs of the individuals and program.

  • Attendance : Adhere to UDI's attendance policies and procedures and have a good and consistent attendance record. Consistently be at workstations and ready to work by the start of the assigned shifts. UDI's normal working hours are from 7 : 30 AM to 3 : 30 PM, Monday through Friday. Other hours and days are assigned based on company need and program requirements.
  • Client Focused : Anticipate, understand, and respond to internal and external threats to client services while ensuring a rounded service offering to include habilitative activities.
  • Confidentiality : Compliance with the agency policies and procedures in relation to HIPAA regulations.
  • Creativity : Strive to provide creative avenues for UDI service recipients to express themselves. Also, being creative with application of the curriculum.
  • Dependability : Consistently follow through with assignments in a timely and effective manner.
  • Effective communication : Speak, Listen and write in a clear, thorough and timely manner using appropriate and effective communication tools and techniques. Communicates. With other staff and supervisor(s) on progress, needs, concerns and changes.
  • Foster teamwork : Function independently. As part of a larger team of professionals. Maintain a team mindset through being collaborative, cooperative, and respectful.
  • Problem solving : Assess problematic situations to identify causes, gather and process relevant information, generate possible solutions and make recommendations and / or resolve the problem.
  • Service line : Knowledgeable about service line and variations in service delivery. Familiar with staff to individual ratio and hours of services needed to maintain individual services based on service definitions.
  • Safety : Consistently perform their jobs in a manner that puts safety first for themselves, coworkers, and clients with no accidents or incidents (Inclusive of property damage or near misses).
  • WORK ENVIRONMENT :

    This job requires physical effort and operates in an environment that will include heat, cold, dust, loud noises, and moving equipment. Aspects of the position may include transportation, community outings, and / or home settings for individuals served. Job may also require travel to individual's home and community services locations for meetings / supervision. Job may require you to work under the following conditions :

  • Sit for long periods of time.
  • Stand for long periods of time.
  • Lifting at least 25 lbs., pushing, pulling, bending, and / or squatting.
  • Potential exposure to aggressive behaviors Inclusive of physical and verbal aggression.
  • Clothing may get dirty and / or torn.
  • Possibility of cuts, scrapes, bruises, burns etc., related to contract work and rehab activities.
  • QUALITIFICATION :

  • Effectively speak, read, and write in English.
  • Must be able to handle physical demands of the job.
  • Must be able to pass all employment screenings.
  • Musical / arts / crafts / teaching or therapy backgrounds preferred.
  • Must have a valid driver's license and meet requirements of insurability by UDI's insurance company.
  • Documented experience with the population served, preferred.
  • Four-year college degree in the field of Human Service and a demonstrated knowledge of appropriate treatment modalities with at least one year of experience in direct service delivery to adults with developmental disabilities; or a two-year degree in a Human Service field, which includes a practicum with a similar population served and at least two years of experience with the population served. Degrees must be from a United States Department of Education recognized accredited college or university.
  • Additional relevant qualifications are preferred.
