Overview
The Community Outreach/Social Services Assistant is responsible for outreach to community partners and assisting the Case Manager with discharge planning and obtaining resources for patients.
Responsibilities
- Organize, administer and facilitate adolescent health programs in the local schools.
- Engage with patients to assess their needs and provide appropriate resources or referrals.
- Support fundraising efforts through event planning, outreach, and community engagement.
- Conduct public speaking engagements to raise awareness about social services available in the community.
- Manage social media platforms to promote programs and share success stories.
- Collaborate with community organizations to enhance service delivery and outreach efforts.
- Maintain accurate records of patient interactions, program participation, and outcomes.
- Participate and represent the hospital at community events such as health fairs.
Experience
Candidates should ideally have experience or skills in the following areas:
- Fundraising: Experience in organizing events or campaigns to raise funds for social initiatives.
- Recruiting: Ability to attract and engage volunteers or participants for programs.
- Public Speaking: Comfortable presenting information to groups and engaging audiences.
- Social Media Management: Proficient in using various platforms for outreach and communication.
- Social Work: Understanding of social work principles and practices is beneficial.
- Public Health: Knowledge of public health issues relevant to the community served.

Qualifications
