JOB DESCRIPTION - Licensed Clinical Social Worker
Position Summary
The Case Management Social Worker is responsible for a range of assessments measuring patients' psychosocial needs in the ED, Med / Surg, and L / D by physician or nursing staff. This position partners with medical staff to assist patients and families in order to manage care and achieve an effective transition from hospital to post-hospital care. By assuring quality care is provided to patients and assisting in overcoming barriers, the Social Worker is an active participant in the process to improve patient outcomes and operational efficiency.
Roles and Responsibilities
- Provides patient and family counseling, psychosocial assessments, evaluations, and crisis intervention by referral
- Collaborates with interdisciplinary team to facilitate timely high quality care as well as appropriate discharge or transfer
- Documents all team, physician, and patient or family communication and concerns pertaining to coordination of care and additional services
- Maintains accurate and current notes in the patient medical record and produces reports of evaluations or treatment as required
- Assists reporting and standards of practice review for social work services
- Promotes continuity of care by accurately and completely communicating to other caregivers the status of the patient
- Communicates relevant elements of the health plan benefits
- Adheres to the Organizational policies and procedures
