THIS POSITION IS UNDER DIRECT SUPERVISION OF THE DIRECTOR OF NURSING.
QUALIFICATIONS / EDUCATION :
Registered Nurse currently licensed by the State of Georgia.
Must have MDS experience
The MDS Resident Care Coordinator shall maintain resident care plans and perform resident MDS assessments, ensuring compliance with established OBRA standards and guidelines and regulatory requirements by all nursing personnel. The MDS Coordinator must be educated and credentialed in age specific needs of adults and geriatrics as well as physical, psychosocial and cultural needs.
The MDS Resident Care Coordinator must demonstrate a high quality of work and commitment to accomplishing the functions of the Department of Nursing. Will act as a resource to families, residents and staff in matters related to Care Plans. Will work with Supervisors and Nursing staff to identify changes in resident status and / or change in condition and document and report changes to physicians and families as indicated. Must maintain professional growth and keep abreast of nursing and health related fields.
