POSITION SUMMARY:
The Case Manager - Support Services role interfaces with UGMTC programs to provide case management, consultation, and referral services for residents and guests. With limited supervision, this role also leads our QUICK-Care case management sessions and conducts needs assessment/goal planning services. This role is critical in connecting UGMTC residents and guests to all relevant resources.
DUTIES AND RESPONSIBILITIES:
The Case Manager - Support Services role interfaces with UGMTC programs to provide case management, consultation, and referral services for residents and guests. With limited supervision, this role also leads our QUICK-Care case management sessions and conducts needs assessment/goal planning services. This role is critical in connecting UGMTC residents and guests to all relevant resources.
DUTIES AND RESPONSIBILITIES:
- Exercises independent judgment in developing and tracking case management plans, providing consultation and referring to appropriate internal/external providers and human service agencies
- Adheres to necessary data entry practices, effectively meeting departmental objectives
- Assists Support Services Supervisor in the implementation of program policy and procedures, measures effectiveness of services, and recommends improvements consistent with the ethical standards of the profession
- Hosts UGMTC QUICK-Care case management sessions for residents and guests in Emergency Services and UGMTC housing programs, providing resources and referrals as appropriate
- Uses established case management methodology and practices to establish and monitor resident goals and case management plans ensuring resident confidentiality
- Collaborates with Program Directors, Program Chaplains and Mental Health Counselors, intake specialists and other program managers on case issues, and resident/guest progress, to support spiritual development, coordinate care for residents, and make program recommendations
- Assists with coordinating volunteers in Support Services
- Builds relationships with key community stakeholders
- Maintains cooperative relationships and communication with other departments to ensure access and positive experiences for UGMTC residents and support achievement of program objectives
- Meets appropriate ongoing training objectives established per departmental standards
- Acts as Task Supervisor for interns volunteers as needed, in limited capacity
- Maintains HMIS licensure as needed, including necessary training updates
- Demonstrate and uphold professional boundaries and ethics when working with residents and guests.
MINIMUM JOB REQUIREMENTS:
- Bachelor's degree in Social Work, Psychology, Human Services, or other related field or completion of degree within 3 months of hire or minimum 3 years of commensurate experience
- Professional licensure: As required (per degree type, governing board, and MN statutes)
- 1 year case management or social work experience preferred
- Experience working with individuals experiencing mental health issues, homelessness, poverty, addiction or brain injury
- Knowledge of County, Department of Human Services, Ramsey County Coordinated Entry and St. Paul permanent-supportive or permanent housing resources preferred
- Proficiency in a second language (Hmong, American Sign Language or Spanish) preferred
- Valid driver’s license, insurability, and ability to transport clients to community resources, as needed

WORKING CONDITIONS AND PHYSICAL EFFORT:
- Work is normally performed in an office within an emergency shelter/transitional housing environment
- May require some evening/weekend hours
- Minimal remote/hybrid potential, up to 20% of work time, with advanced supervisor approval
