Position Title: Care Coordinator

Program: Counseling and Treatment Center

Classification: Non-exempt, Full-Time

Salary Range: $24.00 to $26.50/hr


Summary and Range of Authority: Counseling and Treatment Center, is based on a holistic approach to service delivery with an understanding that adults, TAY and older adults with serious mental illness can benefit from linkages to primary care physicians, access to non-traditional and culturally accepted healing communities, family and peer support, culturally appropriate education about mental illness and substance use, employment support services, and skill development. 


Primary Duties/responsibilities: 

· Care Coordinator will serve to engage in warm handoff of individuals coming into the program from sources such as hospitals, crisis stabilization units, jails, or stepdown from other community programs as well as provide warm handoff of clients to community services and supports for step down from our program. 

· Services include but are not limited to discharge planning, coordination with PCP or other healthcare services, referrals and coordination with outside psychiatrists or therapists, coordination with hospital social workers and any other parties involved in the care of the individual. 

· Care Coordinators may hold a caseload of approximately 60 clients; conducts Annual Update; completes Outcome Measures; conducts rehab groups. 

· Conducts Individual Rehabilitative services or counseling services as needed; embraces philosophy of behavioral health integration with primary care; and takes steps to transfer stable clients to primary care settings when appropriate

·  Completes discharge summaries for any clients who are ready to discharge from the program; functions as a member of a multi-disciplinary team and participates in diagnosis, treatment and discharge planning

· Prepares medical records of all services rendered including psychiatric history, mental status examinations, progress notes, treatment plans, outcome measures, etc.

· Performs case management services as needed

· Monitors clients for any high-risk factors on a regular basis

· Monitors clients for any primary care health needs which require medical attention. 

· Works collaboratively with and receives support from other members of the team to manage clients that are at high risk or in immediate crisis

· Informs Program Manager immediately if a client is in crisis

· Provides interpretation and cultural broker services between clients and staff therapist, psychiatrist and nurse to include verbal and written translation as needed, including in the office and occasional home visits

· Participates in staff meetings, training and quality assurance activities as needed

· Conducts outreach at community health fairs or ethnic community fairs at least 1x per quarter which may include possible evenings or weekend events

· Maintains productivity requirement of 60% billable and non-billable services per week. 


Requirements

Minimum requirements: 

· Minimum Bachelor’s degree in social work, psychology, counseling or human behavior from accredited college or university with major work in clinical therapy

· Minimum of two years of clinical experience in a mental health setting

· Trained and experienced in diagnosing clients utilizing the DSM-5

· Knowledge of the theory and practice of psychotherapy and the principles and practice of community mental health. 

· Bilingual/bicultural in threshold language preferred


Additional preferred requirements: 

· Familiar with community resources and the delivery of case management services

· Ability to establish and maintain diagnostic and psychotherapeutic relationships with clear understanding of ethical and professional standards

· Ability to maintain strict confidentiality with regard to client information and other sensitive or proprietary information

· Ability to communicate mental health principles to clients, families and providers’ ability to work independently and also collaborate as a member of an interdisciplinary team

· Ability to work independently and also collaborate as a member of an interdisciplinary team; excellent written and verbal communication skills

· Ability to prepare written reports accurately and records of services rendered in a timely fashion. 

· Will not be debarred or excluded from participation in Federal programs by the General Services Administration and/or the Department of Health and Human Services of the Inspector General; and must pass background check (Live Scan Fingerprint).


Physical Demands and Work Environment:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Spends approximately 70% of work time sitting and meeting with others or working at a desk and/or computer. Spends approximately 30% of work time standing or walking within the work area or outside the office at meetings and events, also bends, twists, stoops and reaches. Ability to communicate in writing and verbally. Regularly required to sit and talk or hear. Frequently is required to use hands to touch, handle or feel and reach with hands and arms. Occasionally lifts and/or moves up to 25 pounds. Working conditions are normal for an office environment and event venues. The noise level is usually moderate.
