Summary

As a healthcare professional at Unique Home Health Care, you will play a vital role in delivering personalized patient care. This position is essential in supporting our mission of providing high-quality health services that cater to both medical and non-medical needs.

Responsibilities

• Develop and implement individualized care plans based on patient assessments.
• Provide direct patient care, ensuring comfort and safety during all interactions.
• Assist with discharge planning to facilitate smooth transitions from hospital or clinic settings.
• Monitor patient progress and adjust care plans as necessary to meet evolving needs.
• Educate patients and families about health management and available resources.
• Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
• Maintain accurate documentation of patient information and care provided.

Qualifications

• Proven experience in patient care, preferably in home health or similar settings.
• Strong understanding of medical terminology and healthcare systems.
• Experience with developing care plans tailored to individual patient needs.
• Knowledge of geriatrics and pediatrics is a plus but not mandatory.
• Excellent communication skills for effective interaction with patients and families.
• Ability to work collaboratively within a healthcare team environment.
