Job Description: Works with children and adolescents (C&A) with mental health and substance abuse diagnoses in schools, community settings, and home environments. Facilitates and leads skills building groups. Works with youth, families, and school staff for early intervention and promoting positive behaviors in a variety of settings. Responsible for care coordination and addressing barriers. Provides ongoing support for clients in after-care services. Assist with transportation via 15-passenger van as needed to facilitate service delivery. Obtains service authorizations. Position is part of the Rural Communities Opioid Response Program (RCORP) grant awarded to Unison BH via the Health Resources and Services Administration (HRSA). Other duties as assigned. Position is part of a 4-year grant.
Schedule: Will work Monday – Friday, 11 AM – 7 PM.
Competencies: Knowledge of community resources and connections. Knowledge of recovery process and symptom management. Good organizational, interpersonal and communication skills are a must. Knowledge of basic computer skills.
Required Minimum Qualifications: HS Diploma/GED and 3 years of applicable experience. Must be at least 21 years old to apply. Must possess a valid driver’s license and have a clean driving record with less than 6 points.
