What are the responsibilities and job description for the Program Specialist position at United Care Community Home LLC?
A Program Specialist is responsible for the following:
- Supervises Program Managers and reports to the Assistant Director or the Executive Director.
- Coordinates services and completes assessments for adults with developmental and intellectual disabilities (as required under §6400.181).
- Implements the ISP as written and attends ISP meetings.
- Fulfills the role of plan lead relating to development, annual update and revision of the ISP, and ISP review and revision for content accuracy. (Required under §6400.182 and §6400.186).
- Reports content discrepancies to the Support Coordinator (as applicable), Direct Support Professionals, supervisory staff, and management.
- Supervises monitors and evaluates services provided to the individuals.
- Documents the review of the ISP and provides the documentation for the ISP review to the Support Coordinator (as applicable), and plan to team. Informs team members of the option to decline the ISP review documentation (required under 6400.186).
- Reviews the ISP with the individual.

- Coordinates all services (including behavioral supports) provided to the individual in alignment with their ISP and recommends revisions to services or outcomes identified in the ISP, as needed, to the SC and team members (provided under §6400.186).
- Reviews, signs, and dates the monthly documentation of an individual participation and progress toward outcomes. Following up with the program manager to ensure that the Therap notes are in. Audits the quality of Therap notes to ensure they meet all ODP requirements.
- Oversees the quality management of the Individuals’ services and ensures staff adherence to company policies and ensures that progress notes (Monthly and Quarterly) are written in detailed for all service deliveries and submitted every 3 months and submits to the program director for review and then submits to the SC.
- Assesses, counsels or trains Individual in the management of their finances.
- Makes all the arrangements with emergency shelters, provides staff with evacuation plans, and facilitates the evacuation of staff and Individual in emergency situations.
- Arranges for medical care, counseling, or victim’s assistance program in the event of an unusual incident.
- Participates in the on-call supervisor rotation.
- Attends staff meetings and other meetings as required.
Community Participation Support:
- Supervises and directs CPS Direct Support Professional.
- Supervises Direct Support Professionals to help facilitate an individual’s inclusion in the community through experiences and activities.
- Create activity plans for the individuals, making sure they participate in the activities they enjoy.
Working Conditions:
- Responsible for covering shifts, as necessary.
- On-Call 24 hours a day, in case of an emergency, in cooperation with other management staff.
- Ability to lift a minimum of 50 pounds.
- Work will take place in the administrative office, community and in the homes.
- ODP/ Departmental Trainings, HRST Rater, Able to access HCSIS and EIM
- Other Tasks Assigned.
A Program Specialist must have one of the following:
- A master’s degree or above from an accredited college or university and 1-year work experience working directly with individuals with an intellectual disability.
- A bachelor’s degree from an accredited college or university and 2 years work experience working directly with individuals with an intellectual disability.
- An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism.
Program Specialist qualifications:
- Must be 18 years or older.
- Valid driver’s license.
- First Aid and CPR certification, NADD Dual diagnosis certification.
- Trainings on areas of intellectual disability, principles of integration, rights and program planning and implementation, and fire safety protocol and procedures.
