BASIC FUNCTION: Provides case management/service coordination, advocacy, support, personal care, and work-related training and community integration opportunities for clients enrolled in OakLeaf Day Services. Develops and implements person-centered goals for individual plans. Observes, assesses, instructs and monitors progress of individual clients by spending time in the activity rooms with them. If needed, implements behavioral supports. Encourages and models appropriate work-related behaviors. Interacts cooperatively with UCP staff, referral sources, family/guardians and other stakeholders to promote and demonstrate a team approach to service delivery and integration.
RESPONSIBILITIES:
Tasks Supervised by the Day Services Manager:
1. Gather and coordinate data, observations and client’s, guardians’ and/or family members’ expression of needs and wants to complete OakLeaf assessments and develop action steps and procedures that assist the individual to complete the desired outcome of their Individual Service Plan (ISP). Attend annual ISP meetings in cooperation with the Day Services Manager, Day Services Supervisor and outside agencies according to this Agency’s procedures.
2. Serve as an advocate on behalf of clients assigned to caseload as well as all UCP clients. Act as lead in resolving issues and crises related to optimal program participation.
3. Identify needs and initiate appropriate referrals for ancillary services as needed.
4. Work cooperatively with relevant parties to recommend and implement creative programming opportunities to enhance the variety of experiences and activities available to OakLeaf Day Services clients.
5. Document services provided in compliance with agency policies and procedures, Medicaid regulations, CARF standards, and funding requirements—i.e. Individual Service Plans, case notes, referrals, incident reports, data sheets, etc. Review monthly data sheets for accuracy and completeness. Submit for billing in a timely fashion.
6. Attend all agency meetings, department staff meetings, and special team or committee meetings as required and as requested.
7. Perform other reasonably related job duties or assignments as requested.
Tasks Supervised by the Day Services Supervisor
1. Directly observe, assess and work on-site with clients assigned to caseload.
2. Work in close cooperation with the Day Services Supervisor in the training of activity room personnel with regard to effective client habilitation techniques and the implementation of individual client objectives and behavior support strategies.
3. Perform duties of Day Services Community Connectors and assist with service delivery when needed (e.g. ADL’s, habilitation programming, bus loading/unloading, etc.).
4. Attend all Day Services classroom meetings as required and as requested by the Day Services Supervisor.
Qualifications
QUALIFICATIONS:
1. Combination of education and experience usually represented by a bachelor’s degree in a human service, special education or vocational rehabilitation field, or a minimum of two year’s experience working with adults with developmental and/or physical disabilities, preferably in a day programming capacity.
2. Experience in functional assessment and case management strongly preferred.
3. Must possess effective verbal and written communication skills, computer literacy, and the ability and willingness to function as an active team member.
4. Physical capacity to perform lifting (minimum of 50 lbs.) and assume body positions related to routines of personal care tasks (e.g. pushing, pulling, bending, twisting, reaching, crouching, stooping, kneeling).
5. Possess a valid Ohio driver’s license with no more than two (2) moving violations and have reliable daily transportation.
