BASIC FUNCTIONS:

Responsible for providing on-site training, supervision and support to high school student interns participating in the Summer Youth Work Experience transition program. The Job Coach serves as a liaison between the host employer, vocational rehabilitation, and UCP to promote the agencys mission and to facilitate the integration of persons with disabilities into the community work setting through job training. This includes: task analysis for a particular job; teaching participants the necessary steps required to complete job tasks; working side-by-side with the participants to ensure all aspects of the job are done to the employers satisfaction; using appropriate learning strategies and identifying necessary accommodations..

RESPONSIBILITIES:


1. Provide individualized training, supervision, and support for participants at the worksite which includes: hands-on job coaching, building natural supports, identifying job accommodations, assistive technology, etc.

2. Perform task analyses at designated job sites to learn the specific tasks participants are expected to master.
3. Teach participants the necessary steps required to complete job responsibilities, including (but not limited to) training in vocational, social & interpersonal skills, safety, problem solving, work culture, etc..
4. Work side-by-side with participants to ensure all aspects of the job are done to the employers satisfaction, keeping in mind quality and production requirements and safety concerns.
5. Communicate effectively with OakLeaf Partners management and staff, employers, family members, OOD and County Board personnel to coordinate services, share information and problem solve issues related to performance of persons served.
6. Maintain all required documentation of services, including detailed, accurate and timely verbal and written reports on participant progress, attendance, incident reports, etc..
7. Coordinate and lead end of service meetings including all team members.
8. Provide travel training to job site if necessary.
9. Work cooperatively and proactively with OakLeaf Partners management and staff to ensure individuals needs are met in compliance with agency policies and procedures, CARF standards, funding source and the host employers rules, regulations, and expectations.
10. Act as a liaison between the host employer and UCP to promote a positive image of the agency, its mission, and its services. Promote a productive working relationship to ensure the successful integration of the program participants into the work site culture.
11. Perform other reasonably related job duties as requested.

REQUIREMENTS:
1. Minimum of a high school diploma or GED required; and one year of related experience working with individuals with developmental and/or physical disabilities preferred.
2. Great flexibility in accepting a wide variety of assignments at employment sites in the community, including, but not limited to clerical, manufacturing, guest services, food services, and janitorial settings.
3. Ability to work flexible shifts and hours based upon individual work schedules. Demonstrated reliability and dependability.
4. Uses standard English in oral and written communication.
5. Has professional appearance, attention to detail, accuracy and ability to plan and organize work efficiently and independently.
6. Ability to maintain a positive attitude toward persons served and coworkers, and to work cooperatively with employers.
7. Ability to effectively use computer applications including Microsoft Outlook, Word and Excel required.
8. If applicable at a particular worksite, physical capability to perform lifting (at least 50 lbs.), pushing, pulling, bending, twisting, crouching, stooping, kneeling and reaching.
9. Reliable transportation, a willingness to travel throughout the Northeast Ohio region, and a valid Ohio drivers license with no more than two (2) moving violations.
